Health Insurance Benefits – Blue Cross and Blue Shield
Affordable health care is available through Blue Cross and Blue Shield. Employees are eligible on the 1st day of the month following 30 days of employment.
Dental Benefits – Dental
Our Dental plan is offered to eligible employees at competitive rates. Employees are eligible the first day of the month follow 30 days of employment.
Life Insurance – Standard Life Insurance Company
Employees are provided with basic Group Term Life Insurance at no cost. Accidental Death & Dismemberment Insurance, supplemental and dependent coverage is available at competitive rates. Employees are eligible on the first day of the month following 30 days of employment. Additional life insurance can be purchased by the employee.
Profit Sharing Plan – D.A. Davidson Services.
Employees participate in a Pension Plan funded entirely by the physicians. Employees are eligible after one year of continuous service and attainment of age 21. Employees are vested 20% annually based on eligibility for contributions. D.A. Davidson currently manages the Profit Sharing funds.
Short Term Disability – Colonial
Short Term Disability insurance is available through Colonial Insurance at competitive rates Employees are eligible on the first day of the month following 30 days of employment.
Flexible Spending Accounts – Administered by Paychex
Flexible spending accounts allow employees – on a voluntary basis – to set aside a portion of their pre-tax income to offset the cost of approved health care or dependent care expenses. Employees are eligible on the first day of the month following 30 days of employment.
Paid Time Off
The company pays for time away from scheduled work-time (e.g., vacations, illness). Full-time and part-time employees are eligible after 90 days of employment.
The company pays for 6 corporate holidays. Employees are eligible after 90 days of employment.
Leaves of Absence
The company supports approved leaves of absence, including jury duty, military leave, and Family and Medical Leave (FMLA).
