Profit Sharing Plan - Smith-Hayes Financial Services Corp.
Employees participate in a Pension Plan funded entirely by the physicians. Employees are eligible after
one year of continuous service and attainment of age 21. Employees are vested 20% annually based
on eligibility for contributions. Smith-Hayes currently manages the Profit Sharing funds.
Short Term Disability - AFLAC
Short Term Disability insurance is available through AFLAC at competitive rates Employees are eligible

Flexible spending accounts allow employees - on a voluntary basis - to set aside a portion of
