and a representative from our office will contact you within 1-2 business days.
If you are an established patient of our practice, call 402-465-4545 and our switchboard will transfer you to

our confidential e-mail system, click and send an e-mail to the Scheduling Department. A Scheduling

Q) Do I need to bring anything with me for my office appointment?
A) Yes. Bring your insurance card, a referral if required from your primary care physician, and additional
test results or pertinent medical records. We also suggest you fill out our Patient Registration Form and
HIPAA forms ahead of time in order to expedite your office visit. Copay, coinsurance and deductibles are
due at the time of your visit. Click, to view and print the forms in pdf format. You may fax those forms to
402-465-9011, attention, Medical Records.
Q) How do I know if I need a referral for my office appointment?
A) Many insurance plans, especially HMO’s, require a referral or written authorization from your PCP
(primary care physician) for each visit. Your PCP should be able to tell you if you need a referral but
ultimately it is your responsibility to know if a referral or written authorization is required before each visit
and to obtain one. For further questions or concerns regarding your referral,
Click here to e-mail our Business Department, or call our office at (402) 465-3636.
Q) How soon should I cancel my appointment if I can’t make it?
A) If you must cancel your appointment, please do so at least 48 hours in advance. You will receive a
telephone reminder call 2 days in advance of your office appointment. If you do not give 48 hours notice it
will be considered a "No Show" and you will be assessed a $25.00 fee.
Q) What is a consultation?
A) A consult is another term for an office appointment. If you are scheduled for a consult, you WILL NOT
be having a procedure that day. You will be meeting with a physician to review your symptoms and history
in order to determine the next appropriate step in your care.
Q) I have seen my doctor at Gastroenterology Specialties in the past, so why is a consult visit needed with the
doctor before I can schedule my procedure?
A) It is important for your doctor to update your medical history and assess you prior to scheduling a
procedure in order to ensure your safety. Also, many insurance companies require a current documented
office visit prior to authorizing a procedure.
